Required Vaccinations

VACCINATION INFORMATION:

USCIS requires vaccinations to be completed based on the CDC’s recommendations.. This is based on each person’s age and the Doctor will review your vaccine records and inform you of any vaccinations that you may need. We carry all necessary Vaccines at our office.

If you were vaccinated but do not have the records, we haver the option of checking your blood to see if you have had the vaccines. There is an additional fee for this blood test and the price depends on which blood test you would need.

Children: All childhood vaccination are required to be up to date.

Adults: Depending on the age of the patient, for more details refer to the list below.

Required Vaccinations

Hepatitis B – 0 through 59 years of age
DTaP – 8 weeks through 7 years of age
Polio IPV – 8 weeks through 17 years of age
Hib – 8 weeks through 6 years of age
Pneumococcal – 65+
Hepatitis – 12 through 18 years old
MMR – 12 months through 49 years of age
Varicella – 12 months of age and up
Tdap – 18 years of age and up

Influenza vaccine – 6 months of age and up

COVID-19 Vaccine – 6 months of age and up

COVID-19 Vaccine for 12 yrs or older- Both doses of Pfizer, or Both doses of Moderna, or 1 Dose of Johnson & Johnson. (NEW REQUIREMENT)
